• 6) Additionally, we will arrange virtual and physical wellness sessions and blood donation camps for your members.
• 7) All services will be offered with prior appointment only and any other Hospital offer cannot be combined with this offer.
• 8) Members will carry and show the prescribed membership ID card issued by the “Shree Palanpuri Sthanakvashi jain Association” for availing Hospital discounts. Facilities mentioned under this arrangement will also be extended to family members (Spouse, Children, & Parents)
• 9) In exceptional cases, in absence of membership card, member shall produce Shree Palanpuri Sthanakvashi jain Association reference letter mentioning his/ her membership of “Association”.
• 10) The offer will be valid for a period of one year from the acceptance of this letter and may be extended for a further period of one year on as mutually agreed.
• 11) All services will be charged as per the prevailing Hospital Schedule of Charges (SoC) and Members shall pay directly for the bills.

• 1) The person should be member of PSJA.
• 2) The person must have the ID card of PSJA.
• 3) The person should be a member of PSJA for at least three months prior to his/her availing discount facility from the hospital.
• 4) The facilities under the MOU can be extended to the close family members( Spouse, parents, children, daughter-in law, grand children only).
• 5) Members will make direct payment to the Hospital, for the bills received from the Hospital by Cash, or Credit/ Debit cards.( Incl. interim bills).
• 6) The hospital agrees to give 10% discount on in-patient bill excluding the charges for implants, pharmacy, medical supplies, diagnostic or OT packagesand diagnostic tests         done outside hospital) .
• 7) The hospital agrees to give 20% discount on out-patient ( OPD) bill excluding diagnostic packages and outside hospital test.
• 8) The hospital agrees to give 12.5% discount on MRI & CT Scan excluding contrast (even though it is outside test).
• 9) If the member happens to be the staff of the hospital or his relatives, they will not be entitled for the above benefits under the arrangements.

• 1) Saifee hospital agrees to give 20% discount on in-patient bill, excluding Doctor’s fees, consumables, medicine implants, and diagnostic tests done outside the hospital.
• 2) 10% discount on health check up packages.
• 3) 20% discount on all diagnostic tests done on OPD basis ( including MRI & CT Scan).
• 4) 50% discount on Yoga.
• 5) 25% discount on Gymnasium.
• 6) 20% discount on Ayurveda.
• 7) The member has to produce the Membership ID card of PSJA.
• 8) The member has to be a member of PSJA, since minimum 3 months.
• 9) Member will make direct payments to the bills issued by the Hospital.
• 10) PSJA will not interfere in the treatment and medical care provided to benefeciaries.

Termination of Contract : For any discrepancies of services or payments, either of the party can give a month notice and terminate the contract.

Modus Operandi in short : Reports would be collected by members from respective center.

• b. 15% Discount on all other inhouse pathology tests
• c. 5 % on all L5 category tests, which are super-special tests (Internationally Outsource tests).
